Patient policies

I am currently credentialed with the following insurances- United Healthcare, Midlands Choice, Blue Cross Blue Shield, Cigna, Medicaid (IA Total Care & Amerigroup), Aetna, TriCare West, and Medicare. I am pending credentialing with Health Partners, Molina.

If your insurance is not listed above and you are unsure if it is accepted, please check with your carrier directly by calling the number on the back of your card. It is strongly encouraged to verify benefits through your insurance prior to your first appointment.

If your insurance is not accepted but you still wish to be seen, the fee scale can be provided during your initial appointment or by calling the office directly. Co-pays will be due at the time of service. 

Refills are not able to be completed outside of regular business hours. Although I do not accept appointments during all weekdays, I will check for medication refills at least once per day. Please allow 48-72 hours for refill requests to be processed. Please request refills in a timely manner to avoid delays. 

I value your time and want to provide the best care possible, therefore if you are more than 15 minutes late for your scheduled appointment time, you may need to cancel or reschedule. Please contact the office directly to cancel or change appointments. 

In-person or telehealth appointments available. Saturday appointments available 3 days per month. Please check with the provider directly to determine weekend availability if needed. 

Children & Adolescents welcome ages 8 & up.

In the event of an emergency or acute mental health crisis please go directly to your nearest Emergency Department or call 911 for further assistance. I cannot be contacted outside of clinic operating hours.
